Techniques to enhance the sensitivity of immunofixation electrophoresis for serum and urine will be described along with the evidence for improvement in detecting monoclonal immunoglobulins in serum and urine. Emphasis will be placed on using undiluted serum, concentrated urine, use of antisera to free immunoglobulin light chains, and immune subtraction. Modifications to the procedure will be covered in detail so that any laboratory performing protein electrophoresis could execute the procedures without the need to acquire any additional equipment or the need for additional personnel or expertise. The modified methods compare favorably with the sensitivity of mass spectroscopy.
